SUCCESSFUL USE OF AN INEXPENSIVE CUSTOMIZED NEW EXTERNAL PENILE SUPPORT DEVICE (EPSD)
The EPSD was presented to 30 patients as a choice for treatment of impotence. 4 patients choose not to participate in the program.
Introduction: Despite a plethora of therapeutic options available for the treatment of male impotence, a percentage of patients still find the choices less than ideal.
Method: The EPSD was presented to 30 patients as a choice for treatment of impotence. The same patients chose not to participate in this program. A penile supporter (EPSD) was tested in the laboratory with the other 26 patients with physical impotence. The device was used on these patients at least 400 times.
Results: There was an initial satisfaction rate of 50% which increased with re-measurements and re-fittings of the EPSD. 7 of the 10 patients were able to use the device with success. The etiology of the impotence was: diabetes (3), venous incompetence (2), arteriosclerosis(1), psychological disorder (2), hormone imbalance (16), premature ejaculation (2), cancer of the prostate (1), Peyronie's Disease (2), kidney failure (1), post traumatic (1), neurogenic bladder (1). This stainless steel Teflon coated EPSD had a partner acceptance 24 of 30, especially with a newly designed lubricant.
Discussion: Fear of injections, hematoma, fibrosis, infection, continued cost, operative procedure, pain, reoperation, unnaturalness, permanency, slough, effect on self esteem, failure of previous methods to satisfy patient and partner, made patient reluctant to utilize one or several of the standard methods available for the treatment of their potency problems. The External Penile Support Device increased length of the penis on an average of 2.5cm to 4.5cm. Arterial Doppler studies pre and post application of the EPSD in 6 of 19 showed slight increase of arterial flow, (post). In 3 of 26 patients ejaculation was initially impaired. Discomfort occurred in 5 of 26 patients, self esteem increased in 17 of 30 patients. Final satisfaction rate after modification and adjustment of the EPSD to the individual patients was 77%.
Conclusion: This simple device may play role in the therapy of impotent patients.
